Healthcare Without Borders – Community-Centred Care for Lasting Change

Background

Kajiado West is marked by vast distances, scattered settlements, and socio-cultural barriers that keep communities from accessing timely healthcare. Mothers deliver at home without skilled attendance, men and morans avoid health facilities due to stigma, and adolescents lack accurate information on sexual and reproductive health.

To address these gaps, Centrumberliner Medical Centre (CBMC) is pioneering a Medical Outreach and Mobile Clinic Program—a hospital-community partnership designed to bring essential health services closer to where people live, work, and learn.

Program Goal

To expand equitable access to quality healthcare through mobile clinics and structured community outreach, while strengthening referral systems to CBMC.

Specific Objectives

  • Deliver preventive, promotive, and curative health services through mobile units.
  • Increase early detection and management of common conditions (NCDs, maternal health, SRHR, infectious diseases).
  • Empower communities with health education and awareness campaigns.
  • Build trust and continuity of care through strong referral linkages to CBMC.
  • Engage partners in CSR, government, and NGOs to co-create sustainable health impact.

The Challenge

  • Mothers still deliver at home without skilled care.
  • Teenage pregnancies remain high, forcing girls out of school.
  • Non-communicable diseases like diabetes and hypertension go undiagnosed until it’s too late.
  • Distance, culture, and poverty keep communities away from health facilities

Our Solution

  • CBMC is bringing the hospital to the people.
  • Mobile Clinics & Ambulances deliver consultations, diagnostics, and pharmacy services to remote villages, markets, schools, homes, and workplaces.
  • Community Outreach provides maternal & child health, SRHR education, Nutritional assessment, NCD screening, mental health support, and dignity kits for girls.
  • Community Health Champions (CHVs, boda boda riders, teachers) are trained to promote health, respond to emergencies, and link patients to CBMC.

Impact Vision (By 2027)

  • Safer pregnancies and reduced maternal deaths.
  • Girls stay in school through access to dignity kits and SRHR education.
  • Early detection and treatment of chronic diseases.
  • Health services accessible to farmers, traders, and morans in hard-to-reach areas.
  • Strengthened hospital–community trust and referral networks.

Why Partner With Us

  • Aligns with Kenya’s Universal Health Coverage and SDG 3 & SDG 5.
  • Delivers tangible, high-visibility impact for CSR and donor partners.
  • Scalable, evidence-driven model for community health.
